We need to keep our records as current as possible, and you can help us. You will find here, downloadable copies of updating pupil information records, the medication consent form and the application for pupil's leave of absence form. We also need to be informed immediately of any changes in address, phone number, emergency contact details and medical updates.
Please ensure paper copies of the relevant form is handed to the school office. In every case, we need an authorised signature to verify the information given.
